Financial Aid SAP Appeal Form | Students may appeal to continue receiving financial aid if their eligibility for financial aid has been revoked due to unsatisfactory Student Academic Progress (SAP). Please review the SAP Appeal Guidelines and Procedures before submitting. |

Grievance Form | Students may initiate a conflict resolution by filling this form. Student can view the steps in the conflict resolution process here. |

Academic Incomplete Request | Notify the Academic Department if an emergency situation prevents you from completing the trimester in a timely manner. |

Intent to Graduate Form | Notify the Academic Department of your intent to graduate and attend a graduation ceremony. Note: Failure to fill out may prevent graduation. |

Internship Agreement Form | Senior students must find a church leader or pastor to supervise their internship. Both the student and supervisor must fill out this form. |

Leave of Absence Form | Fill out this form if an extenuating circumstance will preent you from attending two or more weeks of class, but you intend on returning to complete the trimester. |

Location Transfer Form | Students and Cohort Advisors may fill this out to transfer the student from one SUM cohort to another SUM cohort. |

Mardi Gras – Leave of Absence | If you are an enrolled SUM student who cannot make it to this year’s Mardi Gras Outreach, please fill out this form with documentation, so that it can be reviewed by the Student Ministry department. |

Non-Degree Enrollment Agreement | Fill out this form to be approved to take classes prior to receiving your High School Diploma or GED. |

SOA Evaluation Form | Every term, students should complete this form. Be sure to write in your Cohort Advisors’s name and email so that he/she can complete the second portion of the evaluation. |
